To utilize the Hope House facility and/or participate in activities sponsored by the Hope House, have a parent or legal guardian complete and return the Permission Slip.  Youth in grades seven through twelve are welcome to attend and participate.  Permission Slips may be submitted to hopehouse@hopehouseofwindsor.org or mailed to 1011 Pennsylvania Ave, Windsor, IL 61957.
